Peninsula HealthCare Connection

Peninsula HealthCare Connection (PHC) is a nonprofit corporation that provides a comprehensive range of health care services to the residents and unhoused clients of the Opportunity Center.  PHC operates a clinic that offers primary care, mental health care, case management, and other supportive services to both adults and children.  PHC has a particular focus on providing needed services and support for those with disabilities.  PHC was founded as a cooperative venture among the Community Working Group, InnVision Shelter Network, Stanford University School of Medicine, the Palo Alto Medical Foundation, and the Palo Alto Veterans Affairs Medical Center.
